The Care Navigator Team Lead plays a critical role in supporting patients with chronic headaches and migraines while leading a team of care navigators in a fully remote healthcare environment. This position combines patient-facing responsibilities with people leadership, operational oversight, and continuous process improvement to ensure high-quality, compassionate care.
Responsibilities:
- Lead, coach, and mentor a team of Care Navigators to meet performance and quality targets
- Oversee patient onboarding, scheduling, and follow-up workflows to ensure timely and accurate care coordination
- Conduct patient intake assessments and guide patients through treatment pathways
- Monitor team metrics, provide feedback, and support ongoing training and onboarding
- Collaborate with clinical, administrative, and billing teams to maintain accurate patient records
- Identify workflow gaps and implement process improvements to enhance patient experience
Requirements:
- Minimum of 3 years of experience in patient navigation, healthcare coordination, or medical assistance
- At least 1 to 2 years of team leadership or supervisory experience
- Strong knowledge of healthcare operations, medical terminology, and patient advocacy
- Experience using EHR or EMR systems, scheduling tools, and HIPAA-compliant workflows
- Excellent communication, empathy, organizational, and leadership skills
- Ability to manage priorities and support a remote healthcare team effectively
