Remote Triage Nurse (Full-Time)

The Remote Triage Nurse will provide frontline patient communication and clinical support, handling triage calls, administering care, and supporting clinic operations. Diana Health is seeking a Remote Triage Nurse to provide patient-focused care and act as the first point of contact for clinical communications. This role supports patient triage, delivers clinical assistance, and ensures smooth clinic operations in a collaborative healthcare environment.

Responsibilities:
- Serve as the first point of contact for patient calls and messages
- Provide clinical care including administering medications and injections
- Support minor visits, lab draws, and patient education
- Assist with clinic workflows, documentation, and record management
- Maintain logs, ensure cleanliness, and support operational tasks
- Collaborate with interdisciplinary care teams to ensure quality patient care

Requirements:
- Current certification as a Licensed Practical Nurse (LPN)
- Minimum of 2 years experience in outpatient or clinical settings
- Strong communication, organizational, and interpersonal skills
- Proficiency with electronic medical records (EMR) systems
- Ability to multitask in a fast-paced healthcare environment
- Bilingual skills (Spanish preferred) are an advantage
