Job Description

 

Cencora is seeking a Case Worker to support patient and provider services through its Innomar Strategies business. This role focuses on assisting patients, healthcare professionals, and funding organizations by providing case management support related to pharmaceutical access programs. The position involves coordinating patient cases, providing telephone assistance, and maintaining accurate program documentation to ensure patients receive appropriate therapy support and guidance.


Responsibilities:
- Manage assigned patient cases and support program-related projects
- Provide telephone assistance to patients, healthcare professionals, and funding bodies
- Administer case management protocols including patient monitoring and follow-up
- Document all interactions and case activities in program-specific databases
- Monitor therapy adherence and report therapy disruptions or adverse events
- Participate in training sessions and provide feedback to improve processes and protocols


Requirements:
- Minimum of 1 year experience in customer service or case management roles
- Postsecondary education or equivalent experience in business, healthcare, or related fields
- Strong organizational and multitasking abilities
- Excellent problem-solving and communication skills
- Proficiency in Microsoft Office including Word, Excel, and Outlook
- Ability to manage deadlines and maintain accurate documentation
