Cencora is seeking a CP Nurse Case Manager for an 18-month contract to support patient care coordination and clinical program operations. This role manages assigned patient caseloads, ensuring timely reimbursement coordination, appointment scheduling, and clinical support. The position requires strong patient advocacy, attention to detail, and collaboration with healthcare providers, pharmacies, and internal teams to ensure accurate documentation and quality care delivery.
Responsibilities:
- Manage assigned caseloads and meet inbound and outbound call targets
- Coordinate reimbursement and appointments with patients, pharmacies, and physicians
- Provide clinical support and guidance on enrollment processes and documentation requirements
- Review feasibility of medication administration in clinic or homecare settings
- Maintain accurate and up-to-date patient records within internal databases
- Report adverse events and provide feedback on protocols and process improvements
Requirements:
- Registered Nurse (RN) or Registered Practical Nurse (RPN) diploma
- Minimum five years of nursing experience
- Healthcare and case management experience in hospital, insurance, or biopharma settings
- Strong patient-focused communication skills and ability to handle medical information
- Ability to multitask and work effectively under pressure
- Proficiency in Microsoft Office applications
